Lutheran Social Services of South Dakota-Arise Youth Center/West
Last year, the Outreach Case Manager worked with a 17-year-old youth referred to Arise through a court order. They worked together to plan for discharge and began exploring independent living options. The Outreach Case Manager assisted the youth and his parents with applying to a local independent living program and even transported the youth to the interview. After acceptance, the case manager helped the youth prepare for his new apartment, complete the court process, and arrange a date for moving. On the move-in day, the Outreach Case Manager transported the youth and oriented them to the program. The case manager continues to assist the youth with job searches, housing options, and accessing additional services.
